I think there's a man lying down on his stomach inside of the table and the table only covers up his torso. The legs you see are his real legs. The body on top of the legs is probably another person squeezed inside the top part of the costume. When the assistants moved the top part of the body down, the nurse brought out a different set of legs, which was hollow. When the doctor turned the top part of the body over, the guy squeezed in the top part could extend his legs into the pants.
