She is actually holding the note. The "changeover" you hear is the volume on her mic being turned way up because she drops to a much softer tone so as so be able to sustain the note.

It's funny and a little sad that people think this song was not looped. Of course that note is looped and she is laughing throughout because she knows everyone knows it is looped; she even laughs from her gut and you can see her stomach and diaphragm heave. It was part of the fun. A great singer doesn't need tricks but this was part of the gimmicky nature of the song.

i commented this video months ago insisting that there was no way this was looped...I was wrong.
i don;t know if it's looped but there had to have been something done. You can actually hear a change in note at around 1:30.
